Alcohol questionnaire

This questionnaire provides a simple method of screening for excessive drinking and can be used to identify harmful or hazardous patterns of alcohol consumption. The following questions should be answered in relation to your use of alcoholic beverages over the past year, please answer as accurately and as honestly as possible. You will be provided with a total score, once the questions have been answered, and a guide to what your score can tell you.

By alcoholic beverages, we mean your use of wine, beer, vodka, etc. Some of the questions will refer to 'drinks consumed', in which case one drink can be thought of as a small glass of wine, a half pint of beer, or one shot of spirits.

So that we may gather an idea of the type of person who is completing this questionnaire, please answer the questions below.
